Permission Forms

It is essential we need permission from the owners of the location of filming, and the actor. That works quite well because I am the actor! As well as Asher's parents being the owners.

Eva Pleciak:

I will be playing “Sam” the character in the film.

I will have agreed to the following details regarding consent and distribution. I then signed this form due to legal production reasons.

Mike & Jen Ballantyne: They are the location holders for the film. They have agreed to us using their house for filming purposes. We also outlines the location owner rights that they have.
